Yes, its reward reaches to the deceased, and his sins are forgiven.

عن أنس يرفعه من دخل المقابر فقرأ يسين خفف الله عنهم يومئذ ومن زار قبر والديه أو أحدهما فقرأ عنده أو عندهما يسين غفرله (عمدة القارى شرح بخارى)

عن أنس قال يا رسول الله إنا نتصدق عن موتانا و نحج عنهم فهل يصل ذلك لهم قال نعم إنه يصل إليهم وإنهم يفرحون به كما يفرح أحدكم بالطبق إذا أهدي إليه (رواه أبوحفص العكبري, شامى ص:258، ج:2)

The above-mentioned traditions prove that doing isal-e-sawab by reciting Yaseen etc the reward reaches to the dead.

For isal-e-sawab, they should do monetary things in charity i.e. to feed and clothe poor and offer them money, or perform Haj and Umrah, or recite any surah from holy Quran like Surah Yaseen, Ikhlas, Takathur etc.
